For young individuals aged 16 and up, we offer supported living placements. The primary purpose is to provide safe, high-quality housing for our young people to develop and improve their independence. Diamond Connections helps young people move from adolescence to adulthood by offering a nurturing environment that promotes growth and independence. Each young person has an in-depth examination to determine their ability and ensure they receive treatments tailored to their specific requirements.

All young people will be assigned a key worker who will work with them to complete their assessment needs and plans, reflecting not only the individual’s viewpoints, interests, and priorities but also providing detailed conclusions about their readiness for transition into independence.

Plans for persons receiving long-term care are evaluated at least once a month or if there is a substantial change. Individual assistance plans with SMART goals and targets will be developed for each young person in our care. These will serve as the foundation for assisting children in acquiring necessary independent skills. Our Supported Living service provides single and multiple-occupancy housing for young people who can live independently with assistance. Our Supported Living service places young people in family-style homes with other young people who are ready to live independently with help.
